Help Us Find A Deserving Family this Holiday Season!

Destined for a Dream is in the process of identifying several families from Bucks
County and Philadelphia for "A Christmas Dream." Many families have been destroyed or hurt by the recession,
unemployment, medical issues, family death and a host of other reasons. We believe in the strength of family! For our third year, Destined for a Dream will provide several
families with their Christmas day meal, Toys for each child , and a gift card wreath to purchase additional
items before Christmas day. The families will receive all items on December 17, 2011. Listed below is the information
and criteria required to participate. Please note that all fields are required.

Please provide the following contact information.
- Full names & ages of each family member
- Street Address, City, State, and Contact Phone Number (day and evening)
- Please tell us your family's "Christmas Dream" (if you are nominating
a family, please explain in 300 words or less why this family is deserving of "A Christmas Dream"
- All information submitted is kept confidential
- The deadline for all submissions: December 16, 2011 @12 noon. If selected, all families will be notified
by 6pm on December 16, 2011
- Winners must be able to pick up all
items at Destined for a Dream headquarters on Saturday, December 17, 2011.
(please limit your response to 250-300 words) Please submit all information along with your essay to: empower@destinedforadream.com Contest begins Friday, November 25, 2011 and will end on Friday, December
16, 2011.
In
2010, Destined for a Dream was able to assist 32 families and 78 children for "A Christmas Dream 2010." Special thank you to all who helped
and supported "A Christmas Dream 2010."
